Consent records are versioned with validity period, scope of approval, and any revocations. This ensures full traceability of the legal basis on which data was collected and may be used.

Pseudonymisation, role-based access controls, and logged changes support privacy-compliant data processing. Only approved and purpose-bound records are included in exports.

Demographics and Base Data

Demographic details such as year of birth, biological sex, gender identity, relevant risk factors, and pre-existing conditions form the basis for stratified analyses. Base data is kept clearly separate from longitudinal data to allow changes over time to be represented accurately.

Mandatory fields and standardised answer options reduce ambiguity. At the same time, project-specific extensions are possible when additional cohort characteristics are needed.

Visual Data Entry

Data capture forms are optimized for different input types and reduce error rates through visual guidance. One example is homunculus widgets for documenting tender or swollen joints.

Diagnoses

Vital Signs

Vital signs such as blood pressure, pulse, body weight, height, and temperature are recorded per time point in a structured format. Timestamps and contextual details — such as resting value or measurement position — improve analytical value.

Plausibility checks detect atypical values or missing entries early. For research purposes, longitudinal views and exportable time series can be provided.

Laboratory Values

Laboratory values are recorded with measurement time, unit, reference range, and method. Consistent documentation of units and reference ranges keeps results comparable across sites.

Validations check value ranges and flag unusual patterns without replacing clinical judgement. Historised longitudinal data enables trend analyses across individual visits.

Imaging and Findings

Imaging-related data includes modality, examination date, body region, key finding categories, and references to DICOM or archive systems. This allows structured findings and raw data to be managed together.

Medications

Procedures and Interventions

Diagnostic and therapeutic procedures are documented with date, type of intervention, indication, site, and outcome. For repeated interventions the course can be traced without gaps per patient.

Structured entry screens support consistent terminology and facilitate later analyses of treatment pathways and outcomes.

Clinical Events

Relevant clinical events such as hospitalisations, complications, or adverse events are documented with date, severity, causality assessment, and outcome. This creates a reliable basis for safety and longitudinal analyses.

Questionnaires and Patient-Reported Outcomes

Standardised questionnaires and Patient-Reported Outcomes (PROs) are stored with version, collection time point, and completeness status. Scores and subscales can be calculated automatically and displayed in longitudinal views.

Creation of Medical Reports
